Adv Health Care Directive - SGVCC with West Marin Senior Services

Event Description: 

West Marin Senior Services (WMSS) and San Geronimo Valley Community Center present a pair of workshops for adults of ALL ages to complete their Advance Health Care Directive - or to revise a current one.

Dr Lakes, palliative care physician with Kaiser, and WMSS staff will provide an overview during the first meeting, hand out forms, and step-by-step address each section to complete the forms, and answer questions. Attendees will return with completed forms to the second session. We will witness and finalize the documents. Dr. Lakes applies years of practical experience teaching first year medical students at UCSF how to approach conversations at the end of life. We will discuss the recent End of Life Option.

WHEN: 

Tuesday March 27th and Tuesday April 10th from 7 - 9 pm 

Thursday March 29th and Thursday April 12th from 1:30 pm – 3:30 pm.

 

WHY: 

It is critical that all adults reflect on and determine their wishes for end of life care. Then, they identify an agent to speak for and advocate for them when unable to do so. More than that, however, completing and Advance Health Care Directive enables a person to specify the way they wish to be treated in specific circumstances.

 

Tuesdays 3/27 and 4/10 at 7 pm and Thursdays 3/29 and 4/12 at 1:30 pm. Attend any combination of the series – day or evening.
